Initial Comments:
An offsite follow-up survey conducted on November 24, 2020, December 9, 2020, and January 19, 2021 found that Medallion Health Care LLC, had corrected the following deficiencies cited under 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601 Home Health Care Agencies.
The deficiencies were cited as a result of an unannounced onsite State re-licensure survey completed September 22, 2020.
